or service provided by Kremer Laser Eye Surgery Centers. The process is easy.
Simply enter the information as you are prompted.
You will enter the following information:
- your name, address and credit card information;
- name and address of the person for whom you are purchasing the gift
certificate;
- instruction on where to send the gift certificate;
- message, if any, you want included with the gift certificate;
- and instructions such as "For delivery on 7/10/99", or
"Please phone me when certificate is mailed", or "Send
confirmation and receipt to [address]."
There are several ways to purchase a gift certificate for a specific
amount. You can simply choose a single certificate amount ($10, $25,
$50, $100, $500, $1000, $2500, $3500, $4500 or $5500) by placing that
certificate into your virtual shopping basket and completing the purchase.
You can create a certificate for an amount not listed by adding several
certificate amounts to your basket. For example, to create a gift certificate
for $200, you would simply place two $100 certificates into your virtual
shopping basket and then complete the order information as prompted.

Your order will be processed through a secure shopping site, which
encrypts your information for the greatest security. Your credit information
is as safe as if you had made the purchase in our office. We will receive
your encrypted order and credit information electronically. Once received,
your credit information will be verified and your order reviewed to

us other instructions, we will send a copy of our executed credit slip
and a copy of the order and instructions to your address.

Each gift certificate will bear the name of the person receiving the
gift, the amount of the certificate, and a unique certification number.
It will be placed in a special gift card inscribed with their name,
any message you requested and your name, unless you have instructed
us that the gift is being given anonymously. It will then be delivered
according to your instructions.
